Failures have to be fixed, networks are expanded and technologies change while operations keep running. Depending on the scope, in-house and contractor teams work on electrical installations, at height, on public roads, while traveling and in restricted-access environments. Service deadlines and availability requirements can increase the pressure for a quick fix.
A single organization may include network, maintenance, engineering, customer service, store, operations center and corporate staff. Each environment perceives risk in its own way. When responsibilities and criteria do not cross these boundaries, safety comes to depend on the supplier, the site or the local leader.
Execution is also often spread across many locations and companies. The organization needs to know whether the planned controls remain effective in the field, how deviations are reported and whether lessons from an incident reach those who face similar conditions.
